riding around, you can do it in what ever gear you want. racing, you should be able to pull 2nd, but under more extreme conditions(hills, mud) you may need first. Sometimes i just putput in first to the line, others i lug out in 2nd. a few times, i have parked at the top of a downhill, and because it is downhill, i start rolling and shift into 3rd or 4th because i naturally gain speed going downhill.

2nd if your bike is warm and your racing, other wise just start in 1st, theres no reason not to

Unless your racing start in 1st
if you are racing start in 2nd
the only reason they start in 2nd is so they start with more power. and they go that quick its almost a waste of time and speed starting in 1st.
there is no need to start in 2nd
