Can a 125 2-stroke be set-up for a 220 lb rider?
I am getting back into MX and want to ease my way back in on a smaller, more controllable bike but I'm fat.

Yes, (stiffer springs and possibly a revalve would be needed). The general target weight a stock 125 is aimed at is in the 165 lb range, so without modifying it would likely be too soft.
Also, it would pull you around OK but would not be spectacular performance. Have you considered a 250 2-stroke?

I have been looking at 250 4-strokes as an option. I am looking for the initial low cost and if a stiffer spring and revalve would be required and would likely have lower performance, I may be better off at the 250 level.
I do like the powerband of the 4-stroke 250 (WR and YZ) but rode a KTM XC360 2-stroke and felt that I would be way overpowered. I was told that it may be the same on 1 250 2-stroke.

True, the 4-stroke 250 is closer to the 125 in peak power but has wider (broader) power spread and would be easier to learn on than the 250 2-stroke.

A 250 4st is comparable to a 125 2st. The suspension is set up for the same weight range and the power is about the same. If a 125 2st won't work for you neither will a 250 4st.

The article on the exhibition racer is inspiring but the picture of MJR isn't quite where I have settled yet. There is time still for me to catch up to him, though. And, I'm more of a black spandex guy. The neon's don't look good on me.
All fun aside, are there stiffer rear springs for most bikes or can the stock spring be set for the extra weight. I read something recently about setting "Sag height." For the front forks, that seems to be something that would require a shop to handle. Any thoughts on cost to reset a 125 for the "fat guy?"
43 years old, 6'0" and 220 lb. but still have some life left on me.

i think you wont be impressed with the power your going to get from a 125. that being said, maybe you should buy one, learn on it and sell it or trade up. i wouldn't bother with the suspension mods. heck i weigh slightly shy of 145 and i'm moving up to a 250.

I'm 210lb and I'm not too fond of 125s. A 125 will move you around alright in your back yard, but once you get into sand or hills the engine will be heavily taxed.

In answer to your question:

Now... SHOULD it be set-up for a 220lb rider? NO. lol
